- comments: observed interior of ice machine with some buildup and dirt. Instructed to remove detail clean sanitize and maintain.
22. Proper cold holding temperatures
- comments: noted internal temperatures of the following time/temperature control for safety food items inside the prep cooler to be improper: pork 47.3 beef 51f and steak 71f. Manager immediately and voluntarily disposed of 7 lbs of products worth $50.00 through denaturing process. Priority violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 5 live roaches on premises under the prep cooler in hot foods prep area, in rear on washroom door, and under three compartment sink. Instructed to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed a 1/4" gap at the bottom of the rear exit door. Must provide a tight-fitting seal at all outer openings to prevent the entry of pests.

- comments: observed pots and pans in rear next to upright two door cooler not inverted. Instructed to invert plates, bowls, pots and pans after sanitizing and air drying.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed the interior of the reach in coolers in need of cleaning. Instructed to detail clean.

- comments: observed drainpipe leaking under hand washing sink in staff washroom. Instructed to repair leaking drainpipe.

- comments: observed food debris and spills on the floors and surfaces throughout the facility. Instructed to detail clean under all shelving units, interior of prep dish area, around dish machine, prep tables and all cooking equipment to remove spills, food debris, dirt buildup and/or grease. Instructed to increase cleaning frequency and maintain at all times.
