- comments: observed evidence of rodent activity on the premises. Observed approximately 40 rat droppings on the floor of the rear storage area behind the grocery carts a02a and ao1j. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

- comments: observed cardboard used as liner inside the reach-in freezer in aisle 11, where bags of chicken is stored. Instructed to remove.

- comments: observed 3-compartment sink stoppers missing on the left side compartment in the juicer area. Instructed manager must provide working stoppers at 3-compartment sink.

- comments: observed the bottom of the 2-door cooler in the juicer area to have food splash and debris. Instructed to clean and maintain.

- comments: observed an unused 2-door refrigerator in the juicer area. Instructed to either repair and maintain the unit or remove them from the premises.
