- comments: pic doesn't have a certified food managers certificate must provide and maintain. (priority foundation 7-38-012)
2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (steak,chicken,italian beef,etc)must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)(citation issued)
8. Hands clean & properly washed
- comments: observed poor hygienic practicies by male food handler observed him with cell phone in hand for several minutes and then put phone on prep table and placed on plastic gloves to prepare a food order without washing his hands and then again the same employee went to garbage can and lifted the lid to put trash in with plastic gloves on and went to prepare food again without hand washing. Instructed to wash hands after every possible contamination. Hands were washed during inspection(cos)(priority 7-38-010)(citation issued)

- comments: observed no hand washing signs at hand sinks in prep area & toilet room must provide and maintain.

- comments: must label food storage containers (shakers & squeeze bottles) when food is not in original package.

- comments: observed evidence of appx. 40 or more mice droppings in corner of pallet of cooking oil. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/11/24. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of front west & east doors must make doors tight fitting to prevent rodent entry.

- comments: observed food product stored on floor in walk in freezer must elevate off of floor '6' and maintain.

- comments: must store wiping cloths in a sanitizing solution when not in use.

- comments: observed several garbage cans with food stored inside must eliminate and provide food grade containers. As well as black and yellow large garbage bags being used for food storage inside of walk in freezer must provide food grade bags.

- comments: must eliminate crates used for food storage in walk in freezer ,underneath equipment in rear area and throughout premises and provide adequate shelving units. Must eliminate cardboard from grill table and other equipment must be smooth and easily cleanable surface. Must remove rust from walk in cooler shelving units.

- comments: must repair or replace heavily soiled and worn cutting board on rear prep table.

- comments: must clean excessive grease and food debris build up from interior & exterior of fryer cabinets and wheel bases,prep tables, grill tables, service counter shelving units,all equipment,food storage containers,pipes of cooking equipment.

- comments: must repair or replace leaky pipe at rear hand sink. Must remove tape from faucet of 3- compartment sink and repair.

- comments: observed both outside grease containers encrusted with grease on top of lids and ground has solid grease with trash and debris mixed in must remove grease,trash and debris and clean and maintain area. (priority foundation 7-38-020(b)(citation issued)

- comments: must repair or replace damaged floor tile in dish washing area,damaged wall at drive thru area. Must grout floor in dish washing area and where needed.

- comments: must remove mold build up from wall on exterior of walk in cooler. Must clean dust and dirt build up from walls and ceiling tiles in prep areas. Food splatter from walls throughout.

- comments: observed expired food handlers training must update and maintain.

- comments: observed no food allergen training for food managers certificate posted on site must provide and maintain.

- comments: previous core violations from report # 2572074 3/3/23 not corrected. #49- 4-602. 13 : must clean the interior of all coolers and freezers to remove spills and debris. Also clean the fans and walls inside the walk in cooler. #51-: 5-205. 15 : observed the 3 comp sink stoppers not working(poor repair). Must repair and maintain to be able to set up sink wash, rinse and sanitize. #55- 6-501. 13 : floors need cleaning through out premises especially along the wall base and in corners to remove spills, debris and/or grease. (priority foundation 7-42-090)(citation issued)
