Report date: Nov 29, 20225. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit/diarrhea cleanup kit or written procedures on site at time of inspection. Instructed management to provide. Priority foundation 7-38-005. Citation issued. Employee arrived during inspection wiht a bodily fluid spill kit; kit now on site.
10. Adequate handwashing sinks properly supplied and accessible
- comments: found the rear prep/dish area handwashing sink not maintained. Sink is inoperable; water was turned off below at pipes upon arrival. Must repair. Priority foundation violation 7-38-030(c). Citation issued. Management repaired sink at time of inspection and hot and cold water now provided.
36. Thermometers provided & accurate
- comments: observed no metal stem probe thermometer on the premises capable of taking hot and cold food temperatures. Management must provide a thermometer to take hot and cold food temperatures. Priority foundation 7-38-005. Citation issued. Employee arrived w/ a metal stem thermometer at time of inspection; thermometer now on site.
38. Insects, rodents, & animals not present
- comments: found no pest control log book on site at time of inspection. Instructed to have a pest control log book on site at all times with all necessary documents (idph license, reports, etc. ) and maintain. Priority foundation 7-38-020(c). Citation issued.
40. Personal cleanliness
- comments: observed a food handling employee preparing food without a hair restraint. Management instructed that all food handling employees must wear a hair restraint while preparing food.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed raw meat stored inside plastic grocery bags in multiple coolers. Management instructed to use only food grade storage bags or containers.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the sanitizer (bleach) being used for sanitizing. Informed person in charge a test kit that accurately measures the concentration in mg/l of a sanitizing solution must be provided. Priority foundation violation 7-38-005. Citation issued. Employee arrived with chlorine test strips at time of inspection, test strips now on site.
49. Non-food/food contact surfaces clean
- comments: found heavy dirt and debris build up on the gaskets inside the 2-door prep cooler. Must clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed the base coving strip peeling off the wall in the front prep area near the 3 compartment sink. Must reattach and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: found heavy dirt and dust build up on the ventilation hood filters. Must clean and maintain.
