- comments: observed used cigar on shelving near cold hold unit in 'market' kitchen area. Instructed person in charge to remove, sanitize area, and instruct all employees to maintain all food areas free of tobacco products.

- comments: observed no handwashing sink located within reasonable distance of dishwashing area. Dishwashing three compartment sink is located in seperate room. Instructed person in charge to install handwashing sink with hot and cold running water under city plumbing and maintained with paper towels and soap. Priority foundation violation #7-38-030(c). No citation issued.

- comments: observed multiple cold hold units inoperable and incapable of cooling in west building market food court (hot line cold hold units, salad bar cold hold units, auxillary kitchen cold hold drawers). Instructed person in charge to repair or remove equipment. Equipment tagged by cdph at this time. Refrigerator unit must not be used until reinspected by chicago department of public health (cdph). Priority violation #7-38-005. No citation issued.

- comments: observed dead fruit flies on floor of roll in cooler in west food court where soda is stored. Instructed person in charge to remove flies and sanitize area.

- comments: observed approximately 200 mouse droppings scattered throughout the west building in the following areas: west production kitchen (inside fryer cabinets, behind fryers, in dry storage room behind and on shelving) market kitchen (on stainless steel shelves, inside fryer cabinets, table bottoms, and behind equipment, on shelf holding stainless steel hotel pans) starbucks (on top of boxes stored on floor near mop sink). Instructed person in charge to remove all mouse droppings, sanitize all areas, and inform pest control of need for additional service. Priority foundation #7-38-020(a). Citation issued.

- comments: observed build up of food residue and grime on shelves and cabinets throughout west building food prep areas. Instructed person in charge to clean and maintain.

- comments: observed multiple missing and broken tiles on interior perimeter of walk in coolers in main production west kitchen. Instructed person in charge to repair and maintain smooth and easily cleanable.
