- comments: must show proof of back flow prevetion device on water intake line of ice machine, coffee brewing machines and water intake lines of ice machine/soda dispensing units. Install and maintain. Observed back flow prevention device located above 3 compartment sink with drain directly above far right compartment. Must redirect drain to mop sink or floor drain with air gap if needed.

- comments: observed grout missing on floors in dishwashing area and in dry storage room. Floor must be regrouted and floor needs to be maintained in a smooth easily cleanable condition.
