- comments: observed no original city of chicago food manager certificate on site upon arrival while tcs foods were served. Instructed manager to ensure original copy of city of chicago food manager certificate is on site at all times. Priority foundation violation 7-38-012 no citation issued.
38. Insects, rodents, & animals not present
- comments: observed over 300 rodent droppings throughout the school including: in pre-school classroom on toys, on sleeping beds and behind coolers, in teacher's lounge near microwave, and in kitchen under shelves of dry storage. Instructed manager to service all affected areas accordingly. Pest control service is recommended. Priority violation 7-38-020(a) no citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed toilet in pre-school toilet room with broken chain. The toilet must be flushed by taking off lid. Instructed manager to repair toilet.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside dumpster receptacle with lid unable to close. Instructed manager to ensure all dumpster receptacles have tight fitting lids which are able to close. Priority foundation violation 7-38-020(b).
