- comments: found no employee health policy/training on-site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on-site. Priority foundation violation 7-38-010. Citation issued.

- comments: low temp dish machine in the kitchen, dispensing improper sanitizer solution,0ppm. The unit was in use. Tagged unit 'held for inspection' do not use. Use a three-compartment sink for: washing, rinsing, and sanitizing. Priority violation:7-38-005,citation issued.

- comments: black and pink slime substance at interior of ice machine water reservoir and ice bin panels. Ice is used for human consumption. Instructed to detail clean, sanitize and maintain unit. Priority foundation violation:7-38-005, citation issued

- comments: we observed(approx 60 or more) small flies in both bars, washrooms, dishmachine area, and basement storage area. Instructed to remove, and sanitize mentioned area. Priority foundation violation:7-38-020(a), citation issued

- comments: must store wiping cloths in prep areas in a sanitizer bucket between uses to prevent cross-contamination. Instructed to maintain.

- comments: utensils used for customers must be inverted in prep area

- comments: broken and rusty at bottom of door of walk-in freezer in basement area, repair issue

- comments: no quaternary nor chlorine chemical test strips on site to check sanitizing levels for the bars three compartment sink, three compartment sink in the kitchen, and low temp dish machine. Three compartment sinks and dish machines are presently being in use. Must provide quaternary and chlorine test strips according to sanitizer solution. Priority foundation violation 7-38-005 citation issued.

- comments: excess items and debris under main bar,clean and maintain. Must elevate items from floor in basement also must keep floor dry throughout the basement.
