- comments: found no kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005. Citation issued

- comments: waste pipe above the food and food container in basement area must be encased. Surface must be smooth cleanable and non-absorbent surface. Or relocate items from under the pipes.

- comments: hot water handle leaking at handwashing sink by the three compartment sink must repair pipes under three compartment sink,presently is hold by a red bucket
