- comments: observed rodent poison stored in an open container behind the storage shelf in the kitchen manager's office. Management instructed that all rodent positions must be stored in a covered, tamper-proof container provided by the exterminator. Priority violation 7-38-005.
38. Insects, rodents, & animals not present
- comments: observed approx. 20 rat droppings on the floor behind the storage shelves in the kitchen manager's office. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed several dead insects on the glueboards in the manager's office and on the shelf of the storage cabinets. Management instructed to remove all dead insects.

- comments: observed several women's staff washrooms without a covered waste receptacle. Management instructed to provide in all women's washrooms.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed one of the lids missing from the outside garbage dumpster and the bottom of the dumpster rusted out. Management instructed to replace the garbage dumpster with a rodent-proof dumpster with functioning lids. Priority foundation violation 7-38-020(a).
55. Physical facilities installed, maintained & clean
- comments: observed several openings in the walls around the storage shelves in the custodian's stock room. Management instructed to seal all openings to prevent rodent harborage conditions.
