Details: Observed no City of Chicago Sanitation Certificate displayed on premises at time of inspection. Instructed management a certified food sanitation manager must be present at all times when TCS foods are being prepared. Priority foundation violation 7-38-012, citation issued.
Observed no employee health policy on site. Instructed to maintain signed health policies for all employees on premises. Priority foundation violation 7-38-010, no citation issued.
Observed no diarrhea and vomit clean up kit on premises. Instructed management a diarrhea and vomit clean up kit with all necessary supplies, a sanitizer rated as effective against norovirus, and written procedure must be kept on site at all times. Priority foundation violation 7-38-005, no citation issued.
Observed improper date marking throughout facility. Instructed management all refrigerated, ready-to-eat, TCS food prepared on site and held more than 24 hours must be labeled with date of creation and disposal. Priority foundation violation 7-38-005, no citation issued.
Observed a menu with potentially raw or under cooked food items (eggs) not labeled with consumer advisory disclosure and reminder statement. Instructed management that all raw or under cooked items must be linked to a reminder and disclosure statement on the menu. Priority foundation violation 7-38-005, no citation issued.
Observed approx. 50 mouse droppings on the floor in the furnace room/storage room near HVAC equipment. Instructed management to clean, sanitize, and maintain area. Additional pest control service is needed to eliminate activity. Priority foundation violation 7-38-020(a), citation issued.
Observed half inch gap underneath both exterior door leading out of storage area and kitchen. Instructed to repair and maintain tight fitting doors.
Observed cutting boards with deep grooves and dark stains. Instructed management to resurface or replace and maintain clean, smooth cutting boards.
Observed build up of dirt, grease, and grime on grill storage shelves, prep coolers, freezer, and storage containers. Instructed to clean, sanitize, and maintain.
Observed a build up of grease and dirt on walls and ceilings throughout the rear kitchen area. Instructed to clean, sanitize, and maintain.
Observed build up of grease and dirt on hood and filters over grill and on vent cover in rear kitchen area. Instructed to clean, sanitize, and maintain.
Observed no employee with an allergen training certificate at this time. Instructed management all employees with a City of Chicago Food Sanitation Certificate must complete allergen training and maintain a certificate on site.
