- comments: observed no valid certified food managers certificate posted to view while tcs foods are being prepared,handled and served such as pizza,hot dogs, milk etc. . Must provide and maintain. (priority foundation 7-38-012)

- comments: observed no hand washing signs in 2nd & 3rd fl. Boy's & girl's toilet rooms must provide and maintain.

- comments: observed evidence of appx. 50 or more mice droppings in pre-k room 112-on rug in library area and in between couches and along wall bases,on floor next to kitchen sink,block area rug. Pre-k room 114-housekeeping tool stand,on floor next to tall file cabinet and storage cabinet across from that,on floor behind toilet room door & on floor in between boxes in same area. Also a dead mouse on glue board under computer desk. Must remove droppings and dead mouse clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 12/4/23. (priority foundation 7-38-020(a)

- comments: must provide a splash guard at hand sink next to 3- compartment sink.
