- comments: observed no hand washing signs at hand washing sinks in staff washroom and basement prep area. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor near refrigerator,under shelving units and on mutli-use equipment in first floor storage area, on floor behind cooking equipment and freezer in first floor prep area, on chest freezer,unused equipment and outlets in basement prep/storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in staff and customer washrooms. Instructed manager to provide.
