- comments: instructed to provide handwashing signage at handsink inside men's washroom.

- comments: instructed to provide thermomemeter visible inside all reach-in coolers.

37. Food properly labeled; original container
- comments: instructed to clean and properly label food storage containers identified with common name (flour, rice etc).

38. Insects, rodents, & animals not present
- comments: found over 50 rodent (mice) droppings along the walls and in all corners in the prep/dishwashing area and in storage area, 30 droppings along the walls inside boiler closet behind front counter. Instructed to remove, clean and sanitize the affected areas. Recommend to contact licensed pest control for service. Priority foundation violation 7-38-020 (a) citation issued

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed some food items stord using milk crates as storage. Instructed to properly use raise shelves 6' off the floor for easy access on cleaning.

- comments: men and women washroom door must be self closing. Instructed to install self closing device.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed dried leaves and debris on ground under and around garbage and grease dumpster in rear outside area. Instructed to clean and maintain.

- comments: observed metal sheet behind the main 3 compartment sink and metal drainboard in poor repair with rust and opening. Instructed to repair or replace.

- comments: previous core violations not corrected from inspection report# 2576719 issued on 06-02-2023 priority foundation violation 7-42-090 citation issued #47 - observed multiple cutting boards with deep scartches and stains in the rear prep area. Instructed the person in charge to resurface/replace cutting boards and maintain. #47 - observed assorted food items inside chest freezers and refrigeration units with food stored inside plastic t-shirt bags. Instructed the person in charge to properly store all food items inside approved food grade containers and maintain. #49 - observed the interior surfaces of all reach -in coolers with food debris and spills. Instructed the person in charge to detail clean and maintain. #53 - observed no covered trash receptacle in the womens toilet room. Instructed the person in charge to provide a covered trash receptacle for the womens toilet room and maintain. #57 - observed multiple employees handling and preparing food without food handling training certificates. Instructed the person in charge to ensure that all food handling employees receive food handler training to comply with idph food handler requirements and maintain.
