- comments: the person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued -

- comments: no certified food handler on site while tcs foods are prepared and served to residents (chicken, soups etc). Instructed to provide. A least one certified food manager must be on site when food is handled. Priority foundation: 7-38-012, citation issued.

- comments: no paper towels are provided at the handwashing sink in rear prep. Upon my request manager on duty provide paper towels at said sink. Priority foundation violation:7-38-030(c) citation issued

- comments: food stored in coolers must have a label with common name.

- comments: outer openings are not protected. Rear door with 1/4' to 1/2' gap at bottom of the door. Seal to minimize rodent insect entry.

- comments: we observed live flying insects on-site in different areas: dishroom 10 under the three-compartment sink and on walls. Between racks (5 or more)which racks are stored on the floor, they are used to wash multi-use utensils at high temp dish machines. Four inside soaps and sponge rack above three-compartment sink. Six on the wall above and under the two-compartment sink. At the storage area, three on boxes, and two were on the wall. Four flying insects on the wall in the hallway by the two unisex washrooms. Instructed to remove insects, clean and sanitize area. Priority foundation violation: 7-38-020(a), citation issued

- comments: mineral and black slime build-up at interior of dish machine. Instructed to delimer and maintain
55. Physical facilities installed, maintained & clean
- comments: we observed excess grimes, debris, and water under high temp dish machine, three and two-compartment sinks, along baseboard walls. Also, floor drain under the three-compartment sink in dishroom with excess black slime substance. Clean and maintain floor dry.

- comments: previous core violation not corrected from 12-9-2021, report#2545379 #58)observed allergen awareness certificate not maintained. Instructed manager to obtain allergen awareness certificates. Priority foundation violation:7-42-090, citation issued
