- comments: observed a procedure for cleaning up vomit and diarrhea but no kit on site. Instructed to maintain a kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. No citation issued.

- comments: observed interior of ice machine in basement used to produce ice for human consumption covered in large amounts of pink and black slime. Instructed to remove ice, clean, sanitize, run 2 loads of ice through and remove, and maintain ice machine free of slime. Priority foundation violation 7-38-005. Citation issued.

- comments: observed the following core violations from report #2382844 on 7/13/2020 not corrected: violation # 39 'no splash guard in front bar exposed hand sink and drainboard of bar 3 compartment sink. Must install a washable splash guard. ' and violation #47 ' must not use unprotected wood under coffee machine. All food contact surfaces should be smooth easy cleanable' instructed to come into compliance on previous core violations. Priority foundation violation 7-42-090. Citation issued.

- comments: observed 3 ashtrays filled with stubbed out cigarettes and ashes and detected odor of cigarette smoke in rear garage used for indoor dining and video gambling. Instructed to not allow smoking indoors. Violation 7-32-020(c). Citation issued. Mandatory court appearance.
