- comments: observed accumulated pink slime build up inside 1st (floor right side) ice machine dripping into ice. Instructed manager must clean interior of ice machine by date of reinspection. Priority foundation 7-38-005. All ice inside said ice machine voluntarily discarded.

- comments: observed 1/4 inch gap at front 3506 n clark entrance door sweep and at rear exit door sweep. Must rodent proof said doors.

- comments: floor cluttered/littered in womens employees locker room. Must clean/elevate all items off floor in womens employees locker room.

- comments: no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates.
