- comments: observed water temperature at all 4 hand washing sinks in toilet room ranging from 78. 4f to 79. 1f. Instructed manager that water temperature must be at minimum 100. 0f. Priority violation 7-38-030(c)

- comments: observed an eyewash station combined/ connected with a hand washing sink. Instructed manager that hand washing sinks are strictly for hand washing and the eye wash station must be separated.

- comments: observed interior of ice machine with black slimy substance. Instructed manager to clean effectively and maintain. Priority foundation 7-38-005. Citation issued

- comments: observed sliced tomatoes and chopped furry tomatoes in prep cooler in serving area at 61. 2f. Manager voluntarily discarded all remaining contents in cooler. All tcs foods must remain at 41. 0f or below. Priority violation. 7-38-005 citation issued.

- comments: observed prep cooler in serving area holding at 52. 9f. Instructed manager that all cooling equipment must hold at 41. 0f or below. Tag placed on unit. Priority violation. 7-38-005 citation issued

- comments: observed no secondary thermometers in prep coolers. Instructed manager to provide to properly monitor ambient air.

- comments: observed individual cake slices in front cooler near front entrance with no labels or other identifing information. Instructed manager to label all foods with ingredients, dates etc.
38. Insects, rodents, & animals not present
- comments: observed at least 30 fruit flies in dry storage across from owners offices and at least 15 flies in rear food prep area around dish washing machine. Instructed manager to remove any food that may be attracting flies, contact pest control company for service, clean and sanitize. Priority foundation. 7-38-020(a). Citation issued

- comments: observed several female employees with no hair nets/hair restraints. Instructed manager that all employees work with/near food, in food prep areas must restrain hair.

- comments: observed cardboard used as liners in between foil pans stored in cooling equipment and on shelving. Instructed manager to remove all cardboard liners and maintain.

- comments: observed chlorine test strips intended to be used for quaternary ammonia sanitizing solution. Instructed manager to provide the correct test strips with the corresponding sanitizing solution. Priority foundation 7-38-005 citation issued
