- comments: we observed aprox. 50 rodent droppings at rear prep area: under shelves,on shelves and mobile carts. Instructed to remove,clean and sanitize mentioned area. No gnawed food. Priority foundation violation:7-38-020(a),citation issued.

- comments: a bucket with sanitize solution and linen must be provided at prep tables
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.
56. Adequate ventilation & lighting; designated areas used
- comments: employees personal items stored at rear prep area must be removed. Instructed to provide a separate area/locker for employee personal items. No employee personal items must be stored in prep area
