- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site while tcs foods (milk, eggs) are being prepared handled and served to the public. Instructed always to have a city of chicago food service sanitation manager on site at all times the facility is open and operating. Priority foundation. Citation issued 7-38-012.

- comments: 6-501. 111 observed evidence of rodent activity on premises. Observed about 20 rodent droppings located in the rear under the flour shelves, about 30 rodent droppings located on the sides of the oven equipment by the oven motors, and about 30 rodent droppings located in various areas of the basement. Instructed to clean and sanitize affected areas and recommended contacting the pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 15 observed the front double doors with a more than one inch gap at the bottom of both doors. Instructed to make said door tight fitting and rodent proofed.

- comments: 4-603. 15 observed large clean in place equipment, and pots unable to fully submerge in 3 compartment sink, instructed to provide written procedures for alternative manual warewashing equipment.

- comments: 4-601. 11(c) observed excessive food, dirt, and flour debris on all non food contact surface (ex: all prep tables, all exteriors of cip equipment, vent fans and cooking equipment) instructed to clean and sanitize all.
