- comments: observed rear screen door to be damaged with missing screen at the rear exit of facility. Also observed was a hole at the base of the door threshold at the rear exit door. Instructed manager to repair screen door and repair hole at base to rodent-proof door to exterior.

- comments: observed 15 rat droppings in the uncovered base of the air handling unit located next to the bathroom at the rear exit door. Instructed manager to clean, sanitize, and maintain the area free of rodent droppings. Priority foundation violation, 7-38-020(a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed food storage bins and flour containers to be stored on the floor in the rear dry storage and the preparation area. Instructed manager to store all food items on shelves at least 6 inches off the floor.
43. In-use utensils: properly stored
- comments: observed utensils stored on soiled shelves and equipment in the rear preparation area under tables. Instructed manager to store in-use utensils in a clean and dry location in between uses.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed baking sheets and pans stored on the floor beneath shelves throughout the rear preparation and baking area. Instructed manager to store all equipment inverted and at least 6 inches off the floor.

- comments: observed heavy food debris and accumulation built on the preparation table shelves and rolling pin holder in the rear preparation area. Instructed manager to clean and maintain the areas free of food debris and residue accumulation.
51. Plumbing installed; proper backflow devices
- comments: observed slow draining hand sink in bakery preparation area and bent faucet at 2 compartment sink area to be leaking from the sides. Instructed manager to service sinks to provide adequate drainage to hand sink and to fix leak at 2-compartment sink.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered trash receptacle in both bathroom at the facility at the time of inspection. Instructed manager to provide covered trash receptacle in each unisex bathroom.

- comments: observed heavy food debris, dust, and grease build up in the following areas: floors and walls below table in the main preparation area; the floor below the 2 compartment sink; floor and walls in the rear baking/oven area are soiled with dust and grease build up. Instructed manager to clean and maintain areas free of food debris, dust build up, and grease accumulation.

- comments: observed mop in the 2-compartment area to be stored in standing water between use. Instructed manager to store mop to allow for drying in between use.
