- comments: found no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010. Citation issued.

- comments: found no procedure/plan and kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005. Citation issued.

- comments: no paper towels provided at both handwashing sinks in prep areas and staff washroom. Instructed to provided paper towels in all handwashing sinks. Priority foundation violation:7-38-030(c), citation issued. Paper towels provided during inspection.

Provided on site was a yellow bottle called 'cloralen' (long lasting fragrance), direction at back of the bottle states: for floor,bathrooms,laundry, remove stains. This product is not for sanitizing multi use utensils at three compartment sink. Must provide the correct sanitizer for food utensils. Instructed to provide. Priority violation 7-38-005, citation issued. Plain chlorine bleach sanitizer provided during inspection.
