1. Person in charge present, demonstrates knowledge, and performs duties
- comments: person in charge at present the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued. -

2. City of chicago food service sanitation certificate
- comments: no original and valid city of chicago food service sanitation certificate provided on site for present manager on duty, while tcs foods are prepared(pork, rice, etc. ). At least one certified food manager must be on site when food is handled. Instructed to provide said certificate. Or enrollment to sanitation class priority foundation #7-38-012. Citation issued

38. Insects, rodents, & animals not present
- comments: we observed pest activity on-site at both dry food storage area. Rat droppings were observed under shelves and beside white reach-in coolers. Over 25 rat droppings from both areas. Instructed to remove clean and sanitize mentioned area. Priority foundation violation:7-38-020(a). Citation issued

- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain
