- comments: observed no employee hand washing signage at bar hand sink. Must provide.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed no chemical sanitizer on site during inspection to sanitize equipment and utensils. Instructed to provide and have chemical sanitizer available on site at all times. Priority violation. 7-38-005. Citation issued. Manager left inspection and purchased bleach sanitizer from store.

- comments: observed approximately 140 mice droppings throughout establishment. Observed approximately 100 mice droppings inside of cooking fryer in rear prep area, found 20 mice droppings behind pizza oven, 10 mice droppings found on top of electrical outlet in rear prep area and 10 mice droppings found behind spices dry storage area in prep area. Must clean and remove all evidence of pest activity on premises. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: observed no visible back flow preventer device on espresso machine water line at bar area. Must provide.

- comments: observed water damage on ceiling in women's bathroom above stall on left side. Must repair issue.

- comments: observed last three ceiling lights not working in prep area. Must replace with working light bulbs.
