- comments: noted employee in food prep with long hair without hair restrain working with exposed food. Instructed to wear proper hair restrain at all times when preparing food and working with exposed food

- comments: noted ice build-up inside the walk-in freezer on the floor. Instructed to repair and maintain the condensation units.

- comments: observed outside garbage receptacle lids not tight fitting per city code. Instructed to repair or replace dumpster lid to prevent rodent harborage. Instructed to maintain. Priority foundation 7-38-020(b) citation issued.

- comments: noted the following: floors throughout the premise, especially along with wall base and underneath equipment and racks with dirt, grease, and food debris, walls with food dirt and food spills. Instructed to detail clean and sanitize and maintain clean at all times. Must upgrade housekeeping.

- comments: observed no food handler's training certification from employees. Instructed that all employees must comply with food handler's training by the food handling regulation enforcement act.
