- comments: 2-101. 11 no city of chicago food service manager on-site during the initial inspection while tcs foods were prepared and served. Priority foundation violation 7-38-012 citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: 6-301. 12 no paper towels available at the prep and dishwashing handwashing sinks. Must provide at all times. Priority foundation violation 7-38-030(c) citation issued.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed mice droppings throughout the facility along all baseboards in the rear storage abot 50, front prepare 25-35, dish room, and office 20-30. Noted droppings on the drive-through window sill 20-30 droppings. Pest control log book noted norway rat activity on the service report dated 4-6-22. Priority violation 7-38-020(a) citation issued.

- comments: 4-601. 11(b) all cooking equipment, food containers, prep stations, prep tables, and coolers inside and out with grease and excessive food debris. Must clean all.

- comments: 5-501. 113(b) missing lid for the outside grease receptacle. Open lids on the outside garbage container. Must provide a lid for the grease container and provide tight-fitting lids on the garbage container. Priority foundation violation 7-38-020(b)citation issued.
