- comments: observed person in charge without proof of valid original city of chicago certified food managers certificate during time of inspection while preparing and serving chicken, mashed potatoes, rice, etc. Instructed to provide at least one certificate holder on site at all times while open and operating. Priority foundation violation. #7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no verifiable/ signed employee health policies on site during time of inspection for all food and conditional employees. Instructed to provide and maintain on site for review as required. Priority foundation violation. #7-38-010. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 40-50 small black live flies flying throughout chicken prep area, on cardboard boxes containing sauces, ceiling in rear storage area, on handsink basin in rear prep. Noted approximately 35 small black crawling beetles under 3-compartment sink in dishwashing area. Recommended to contact pest control for service. Must eliminate pest activity on premise and maintain. Priority foundation violation. #7-38-020 (a). Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed accumulation of grease and food debris on side of cooking equipment accross from ice machine. Instructed to clean, sanitize, and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed unused cooling with a buildup of grease and food debris next to warmer across from food prep table. Instructed to clean and or remove to prevent pest activity.
58. Allergen training as required
- comments: instructed certified food manger to obtain allergen training certificattion as required.
