- comments: observed no city of chicago certified food manager on the premises while tcs foods (beef, chicken, beans) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

- comments: observed a 1-inch gap at the middle bottom of the front entry doors. Must seal gap to prevent rodent/pest entry and maintain.

- comments: observed over 30 live black flying insects on the ceiling and walls near the mop sink, ice machines, and walk-in coolers in the kitchen. Instructed management to contact pest control for additional service to eliminate all pest activity from the premises and to clean affected areas. Priority foundation 7-38-020(a). Citation issued.

- comments: observed the 3rd handwash sink from the left in the women's washroom and the 3rd handwash sink from the right in the men's washroom not providing water. Instructed management that all handwashing sinks must provide running hot and cold water. Must repair.

- comments: observed the main bar and the 360 bar with heavy dirt and debris building up on the floors underneath equipment and floor drains. Must clean floors and floor drains and maintain.
