- comments: observed 4 dead roaches on floor in prep area behind 2 door cooler as well as in 3 comp sink basin. Instructed to remove, clean and sanitize said spaces.

- comments: observed excessive food, trash and grease debris built up inside each table shoot located throughout dining room. Said tables are used for cooking guesssst food. Instructed to remove and clean equipment at frequency as needed in order to prevent pest harborage.

- comments: observed large hole in wall behind 2 door refrigeration unit in prep near pop storage. Instructed to enclose and maintain smooth and easily cleanable surface.
