10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at the exposed handsink near the three-compartment sink in the rear warewashing area. Instructed the person in charge to provide handwashing signage at all hands used by food employees and maintain.

36. Thermometers provided & accurate
- comments: observed no thermometers conspicuously placed in the reach-in coolers in the rear prep area. Instructed the person in charge to provide thermometers for all cold holding equipment and maintain.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approximately 10 mouse droppings on the counter behind the pizza warmer in the front dispensing area. Also observed approximately 20 mouse droppings on the floor underneath the dry storage shelving units in the rear prep area. Instructed the person in charge to remove all rodent droppings, clean surfaces, and maintain premises free of pest. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a).

39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler in the rear prep area. Instructed the person in charge to properly store and protect the food items from contamination while stored by elevating at least six inches from the floor and maintaining.

40. Personal cleanliness
- comments: observed multiple food handlers prepping food without hair restraints. Instructed the person in charge to ensure all food handling employees wear hair restraints to prevent contamination and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rusted storage racks in walk-in cooler in the rear prep area. Instructed the person in charge to remove rust and ensure all nonfood-contact surfaces remain smooth and easily cleanable.

55. Physical facilities installed, maintained & clean
- comments: observed some food debris on the floor behind deep fryers in the rear prep area. Instructed the person in charge to detail clean floors and maintain.

58. Allergen training as required
- comments: observed one certified food service manager without allergen awareness training. Instructed the person in charge to obtain an allergen training certificate and keep on premises and maintain.
