- comments: found no employee health policy/training on-site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on-site. Priority foundation violation 7-38-010. No citation was issued.

- comments: black slime substance at interior of ice machine in basement area. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit. Priority foundation violation: 7-38-005, citation issued

- comments: must indicate on menu 'cook to order' phrase for salmon, steaks, etc. Priority foundation violation:7-38-005, no citation issued

- comments: accumulated grease and food debris on walls, utility connections, and equipment surfaces behind cooking line, also inside deep fryers and mounted on wall potatoes slicer. Must clean and maintain.
