- comments: found no certified food manager on site while tcs foods (ham, sandwich, dairy products, fresh fruit juice etc) are being prepared and served. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present at all times when handling tcs foods. 3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: found no employee health policy/training on site. Instructed to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010 citation issued

Instructed person in charge, tcs foods must maintain cold holding temperature of below 41. 0f and hot holding temperature of above 135. 0f. Management discarded the said food products. - comments: found over 100 rodent (mice) droppings along the walls and in all corners in the bakery prep area, underneath 3 compartment sink, 20 mice droppings inside the storage cabinet in rear prep/storage area. Instructed to remove, clean and sanitize the affected areas. Recommend to contact licensed pest control for service. Priority foundation violation 7-38-020 (a) citation issued

- comments: observed open baked pastries for sale not properly protected from public contamination. Noted different types of bread/pastries in an open/exposed baking racks display for sale in the customer area. Instructed person in charge, all open food (bread and pastries) for sale must be properly protected (covered or store inside the bread cabinet).
