- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. - comments: observed no current pest control log book on the premises. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.

- comments: observed an electric fly trap hanging directly above the two-compartment sink and near the shelf where clean dishes are stored in the rear dish area. According to food code: insect control devices shall be installed so the devices are not located in a food preparation area. Must relocate existing fly trap devices in direct violation of this rule.

- comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.
