Instructed all persons in charge must obtain a city of chicago food service sanitation manager's certificate and be on site at all times the facility is open and operating. Priority foundation. - comments: 2-101. 11 observed no city of chicago food service sanitation manager on site while tcs foods (fish) are being prepared handled and served. Instructed a city of chicago food service sanitation manager must be on site at all times the facility is open and operating. Priority foundation. Citation issued 7-38-012.

- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide at all times. Priority foundation. Citation issued 7-38-010.

- comments: 2-501. 11 observed no written procedure nor adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide at all times. Priority foundation. Citation issued 7-38-005.

- comments: 4-602. 11 observed cutting boards and utensils with accumulated food debris. Instructed to clean and sanitize all food contact surfaces.

- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 10 live large flies flying throughout the food prep and customer service areas. Observed about 75 live small flies on boxes by the customer service counter, under the customer service counter and on the walls in the food prep area. Observed about 50 rodent droppings on the lower shelves of the customer service counter, inside of the fryers, and in the toilet room. Observed about 2 live roaches on a bread prep cart. Instructed to eliminate all rodent and insect activity, clean and sanitize affected areas and recommended contacting pest control for an additional service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-501. 112 observed an accumulation of dead insects along the ledge of the front window in the customer service/dining area. Instructed to clean and sanitize.

- comments: 3-304. 14 observed wiping cloths not being stored in a sanitizing solution. Instructed to store wiping cloths in a sanitizing solution at all times.

- comments: 4-101. 19 observed gaskets in the prep coolers in poor repair and with accumulated food debris. Instructed to replace gaskets in poor repair and clean and sanitize.

Observed accumulated food debris inside of the refrigeration units. Observed equipment in the rear food prep area with accumulated grease and food debris. Instructed to clean and sanitize all. Observed stained ceiling tiles in the food prep area. Instructed to replace.

- comments: 5-501. 15 observed outside gabage dumpster ground area not maintained with cardboard boxes and accumulated debris on the ground area around the dumpster. Instructed to remove cardboard boxes and clean ground area. Priority foundation. Citation issued 7-38-020(b).

- comments: 5-502. 11 observed inside garbage cans full of garbage from the day before. Instructed to remove garbage frequently in order to prevent pest harborage.

Observed accumulated food and dirt debris on the floor throughout the food prep area. Instructed to clean and sanitize all.
