- comments: observed no chicago food sanitation manager on site when tcs food (egg foo young 159. 4f)prepared and served. A chicago food sanitation manager must be on site at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued.

- comments: observed cold tcs food stored in reach-in cooler at improper temperature: raw eggs 56. 9f, raw chicken 57. 8f, beef 56. 3f, egg rolls 57. 2f. Instructed manager to discard over 50lbs and $100 of food. All cold tcs food must maintain 41f or below during storage, preparation, display and service. Priority 7-38-005 citation issued.

- comments: observed sliding door reach-in cooler not maintaining proper temperature 58. 1f. Instructed manager to repair, cooler must maintain 41f or below. Cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued.

- comments: observed over 10 live roaches crawling on spray bottle under prep sink, on floor and walls in prep and dish washing areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed bulk containers not covered in rear storage area, food items stored on floor in walk-in cooler and freezer. Instructed manager to cover all bulk containers and elevate all food items six inches off floor.

- comments: observed cutting boards stored on floor in rear prep area. Instructed manager re wash, rinse sanitize and store all clean multi-use equipment six inches off floor.

- comments: observed damaged gaskets on prep cooler doors. Observed rust deposits on shelving units in coolers and bottom of prep tables. Instructed manager to repair and maintain.

- comments: observed accumulated grease, food buildup and debris on interior and exterior surfaces of coolers, freezers, prep tables, shelving units, cooking equipment, bulk containers. Instructed manager to clean and maintain.

- comments: observed accumulated food debris and grease buildup on floors under cooking equipment, food spills on walls prep area. Observed water and debris on floor in basement. Instructed manager to clean floors and walls in all areas.
