- comments: no one on site with city of chicago sanitation certificate or certificate posted. Instructed manager on duty city of chicago sanitation certificate and person must be on site while serving, preping or holding of tcs food products. Such as rice, beef, pork, chicken etc. . . Priority fondation 7-38-012 citation issued

- comments: all bulk food items must be stored in seal containers with commodity name visible. Instructed to correct and maintain.
38. Insects, rodents, & animals not present
- comments: observed rodent activity in basement area approx 30-40 mouse dropping along wall bases, approx 15-20 droppings in pop storage closet, and 15-20 droppings in front counter area. Instructed manager to clean and sanitize all areas. Contact pest control for service. Priority foundation 7-38-020(a) citation issued

- comments: walls in basement must be sealed to eliminate any crumbling of cement. Instructed to correct and maintain.
