- comments: 6-301. 12 observed no hand drying provision located in the toilet room. Instructed to provide a hand drying provision such as paper towels at all handwashing sinks at all times. - comments: 3-302. 12 observed some dry foods out of the original packaging and inside of containers without a label containing the product name (rice). Instructed to label all dry foods that are out of the original packaging.

- comments: 6-501. 111 observed evidence of rodent activity on site. Observed about 30 rodent droppings around the grease trap under the 3 compartment sink, about 20 rodent droppings around the hot water heater, about 15 rodent droppings under the dry storage racks located across from the pizza oven and about 20 rodent droppings scattered in various areas of the food prep area. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 13 observed insect traps hanging from the ceiling in the front and rear food prep areas. Instructed to remove and only allow pest control to set any pests traps.

- comments: 2-402. 11 observed the manager (food handler) with a beard and without a beard guard and without a hair net. Instructed to provide hair nets and beard guards.

- comments: 5-204. 12 unable to locate the backflow prevention device on the ice machine located in the rear. Instructed to locate or provide.

- comments: 6-501. 12 observed accumulated debris on the floors throughout the prep areas. Instructed to clean along the wall and in all corners. Observed accumulate dust debris on the vents located above the pizza oven. Observed an accumulated black substance on the wall by the rear food prep area's 3 compartment sink. Instructed to clean all.

- comments: 6-501. 114 observed baby crib and a baby chair in the food prep area. Manager states "he brings his kids to the facility sometimes. " instructed to remove and only allow employees in the food prep area at all times.

- comments: 2-102. 13 observed no allergen training certificates on site. Instructed to meet requirement for all food service sanitation managers.

- comments: 8-404. 13 observed previous core violation not corrected from inspection report 2600709 dated 8/26/24; 38 - 6-202. 15: the outer front entrance door has a broken door sweep at the bottom; instructed to replace sweep so that door is rodent proofed at the bottom as required. Instructed to correct previous core violation. Priority foundation. Citation issued 7-42-090.
