- comments: observed manager on duty without a valid city sanitation certificate as required.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago certified manager on duty/site while tcs foods are being handled, prepared and serverd such as chicken and fish. Instructed manager on site certificate and person must be on site at all times when tcs food items are being cooked, prepped, served and/or held. Instructed manager to correct and maintain. Priority foundation citation #7-38-012 issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the hand sink in the washroom in poor repair. Must repair and maintain.
38. Insects, rodents, & animals not present
- comments: observed 50 mice droppings scatterd on the floor in the rear storage/prep area. Noted 25 live flies scatterd on the window shade in the front prep area and 40 live flies scatterd in the rear on boxes,walls and racks. Must remove droppings and flies and call pest control for service. Also detail clean all affected areas. Priority foundation ciation #7-38-020(a) issued.

- comments: must clean interior and exterior of all equipment to remove debris.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must detail clean and maintain the washroom fixtures,walls, ceiling and floors.
55. Physical facilities installed, maintained & clean
- comments: instructed to clean and maintain floors through out under around and behind various equipment, clean walls around and behind equipment where needed. Clean ceiling vents and replace stained ceiling tiles where needed.

- comments: observed unused and broken equipment through out the rear and clutter through out the front. Instructed manager to remove/declutter premises to prevent harborage for pest.

- comments: must clean the hood above the cooking equimpent to remove grease and debris.
