1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with foodservice manager training.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified manager on site at time of inspection. Instructed person in charge to maintain original city of chicago certificate as well as certified manager on site at all times. Priority foundation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in rear prep area next to dishwasher with basin full of bottles and gloves. Observed handsink in rear of kitchen area blocked by freezer and garbage can. Instructed person in charge to remove all items and maintain all handsinks to be accessible at all times. Priority foundation #7-38-030(c). Citation issued.

13. Food in good condition, safe, & unadulterated
- comments: observed food in dry storage prep area to be adulterated showing evidence of chewing by rodents(mice). Mouse droppings found inside flour and rice bulk containers. Instructed person in charge to discard and maintain all food to be protected and unadulterated from premises and pest. Manager voluntarily discarded rice at cost of $25. Priority violation #7-38-005. Citation issued

22. Proper cold holding temperatures
- comments: observed tcs food (pork) at improper cold holding tempertature of 54f in dining room self service area. Instructed person in charge that all cold held foods must be at 41f or below. Person in charge voluntarily discarded 5lbs of pork at cost of $40. Priority violation 7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed mouse droppings too numerous to count scattered throughout facility in the following areas: behind shelving and equipment in dry storage area, in corners of kitchen prep area, on equipment on shelving in dry storage area, on boxes in basement storage area, between and behind chest freezers in basement storage areas. Instructed person in charge to remove all droppings, sanitize all areas, and inform pest control service of increased activity. Priority violation #7-38-020(a). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed dead mice throughout basement area. Instructed person in charge to remove all dead mice and sanitize areas.

49. Non-food/food contact surfaces clean
- comments: observed build up of food debris on interior of 2-door cold hold units in storage area. Instructed person in charge to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed holes in walls in basement corners in rear storage room. Instructed person in charge to repair and maintain smooth and easily cleanable for the prevention of pest harborage.

60. Previous core violation corrected
- comments: observed previous core violations from report #2577937 not corrected; #45 found some food items stored in grocery bags in walk-in freezer; instructed to use food grade bags only for food storage. #55 instructed to clean the floor under and around all kitchen equipment and maintain. #56 observed the vents in the hooded system above the cooking equipment in need of cleaning and the vent above the dishmachine is in need of cleaning; clean both. #58 instructed that all city of chicago food service managers must obtain the required food allergen training. Instructed person in charge to come into compliance and maintain. Priority foundation #7-42-090. Citation issued.
