1. Person in charge present, demonstrates knowledge, and performs duties
- comments: 2-101. 11 observed the person in charge without an original valid city of chicago food service sanitation manager's certificate. Instructed all persons in charge must obtain the city of chicago food service sanitation manager's certificate and be on site at all times the facility is open and operating. Priority foundation. Citation issued under violation #2.

2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed the person in charge without an original valid city of chicago food service sanitation manager's certificate. Instructed all persons in charge must obtain the city of chicago food service sanitation manager's certificate and be on site at all times the facility is open and operating. Priority foundation. Citation issued 7-38-012.

5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur. Instructed to provide. Priority foundation. Citation issued 7-38-005.

22. Proper cold holding temperatures
- comments: 3-501. 16(a:2,&b) observed tcs foods at inadequate cold holding temperatures located in the cafe food prep area; observed a quart of half & half at 53f and not stored in a refrigeration unit. Instructed to discard said tcs foods and always maintain all tcs foods at 41f or below at all times. Food handler discard said tcs food during the inspection. Priority violation. Citation issued 7-38-005.

23. Proper date marking and disposition
- comments: 3-501. 17 observed no date labels for tcs ready to eat foods that are cooked and cooled and held in the refrigerator for more than 24 hours such as cooked chicken, cooked potatoes, cooked pasta (all located on in the main kitchen food prep area and breakfast and lunch sandwiches all located in the cafe food prep area. Instructed to provide date labels for all tcs ready to eat foods that are held for more than 24 hours in a refrigeration unit, facility is restricted to hold tcs rte foods for no more than 7 days (day 1 being the day the tcs rte foods are prepared. ) priority foundation. Citation issued 7-38-005.

25. Consumer advisory provided for raw/undercooked food
- comments: 3-603. 11 observed facility with consumer advisory disclosures on items that are not allowed to be served undercooked such as pork and chicken. Instructed to remove the consumer advisory disclosure from all items that can not be served undercooked or raw such as pork and chicken. Priority foundation 7-38-005.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of live insect activity on site. Observed about 5 large flies flying around in the cafe food prep area and about 20 live large flies on the dish racks, on the walls and flying around the main kitchen food prep area. Observed about 30 live small flies on the walls, ceilings and flying around the main kitchen food prep area and about 30 live small flies on the shelves, and under the one compartment sink in the cafe food prep area. Instructed to eliminate live large and small flies actiivty, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed multiple employees state the live large flies and live small flies actiivty is due to the facility opening unscreened windows throughout operation times. Instructed to appropriately screen all windows when opened or do not open unscreened windows in order to prevent pest entry at all times.

39. Contamination prevented during food preparation, storage & display
- comments: 3-303. 11 observed ice used for human consumption in an ice bin with containers submerged in the ice (ice being used as a coolant) in the front cafe food prep area. At the time of the inspection the food handler scooped ice out of the ice bin and used the ice to prepare a drink for a customer. Instructed to never use ice that is used for human consumption as a coolant for containers or any other possible contaminates within the same ice bin. Food handler obtain new ice for human consumption from the ice machine in the main food prep area. Priority violation. Citation issued 7-38-005.

41. Wiping cloths: properly used & stored
- comments: 3-304. 14 observed a wiping cloth stored on the bar (wiping cloth has 2 live large flies and 4 live small flies on it). Instructed to always store all wiping cloths in sanitizing solution at all times.

45. Single-use/single-service articles: properly stored & used
- comments: 4-904. 11 observed plates not inverted and some glassware not inverted. Instructed to store food contact surface inverted in order to protect from any possible contamination.

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed excessive food and grease debris in between cooking equipment and under the flat top grill. Instructed to clean all.

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 observed an out of order sign on one of two hand washing sinks located in the women's toilet room, and tape on top of the one compartment sink in the cafe food prep area, when the water was turned on at the one compartment sink an excessive leak from the p-trap of the one compartment sink occured. Instructed to repair all.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 observed accumulated dust and dirt debris on the walls in the main kitchen food prep area. Instructed to clean.
