- comments: no signed employee health policies on the premises at the time of inspection. Must provide signed employee health policies for all employees. ( priority foundation violation. ) (citation issued 7-38-010). Employee health policies were provided at 10:41 a. M after citation was issued. (cos)

- comments: observed approx. 50 or more mice droppings in various areas of the facility such as, in the kitchen inside of food storage cabinet, the mop sink closet on the lower shelf next to tool caddy and floor tiles, in the 1 southwest classroom lower shelf writing desk cubbies, and in the northwest classroom on the floor in between the cubbies and at the bottom of the file cabinet in front of rear door. Must remove droppings, clean and sanitize all areas affected. Recommended pest control services 6/21/23. ( priority foundation violation. ) (citation issued 7-38-020(a).

- comments: observed gap in the center of front double doors. Must make doors tight fitting to prevent pest entry.

- comments: observed aluminum foil being used as as a liner on the grease trap in the kitchen area. Instructed to remove or replace with something smooth or easily cleanable.
