- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are held. Voided copy of certificate was provided. Informed person in charge that an original city of chicago food service certificate must be posted at all times. Priority foundation violation #7-38-012. Citation issued.

28. Toxic substances properly identified, stored, & used
- comments: all personal articles must be stored in a personal storage area not in prep, dish or service areas. Instructed to provide lockers or personal areas for storage correct and maintain

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity in cafeteria. Observed approximately 20-30 mouse droppings on all shelving under cafeteria counter and in drawers of counter. Observed 15-20 mouse droppings on the floor near the reach in behind garbage. Observed approximately 20 mouse droppings on the floor behind and on lower shelf of prep table where the microwave is stored in cafeterria. Observed about 50-60 ants on floor next to cooking equipment. Observed approximately 5-7 fruit flies in cafeteria. Management instructed to clean and remove ants, fruit flies, and all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed leak at pipe underneath at handsink behind cafeteria line, leak at 3-comp sink at sanitizing sink, and leak at dish machine.

60. Previous core violation corrected
- comments: observed previous core violations not corrected from report #2593568 on 4-29-24 violation #47- cooking equipment such as stoves, steamers, ovens with a build up of dirt, food debris, and food splatter on the sides and in between the units. Violation #49 observed the inside of the walk in cooling units with a heavy build up of dirt, spillage and food debris on the floors under the shelving units. Violation #55 observed a build up of dirt and food debris in the corners, under cooking and noncooking equipment. Instructed to clean and maintain all areas as a means of preventing possible insect and or pest harborage. Violation #55 observed damaged ceiling surfaces in dish washing area and damaged wall surfaces in food service area. Instructed to correct all previous core violations. Priority foundation violation 7-42-090 citation issued.
