2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on site while tcs foods are being prepared such as beef stew, rice,etc. Instructed manager that a certified food manager must be on site at all times. Priority foundation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signage at hand sinks in front prep areas, in dine in area and in toilet room. Instructed manager to provide signage and maintain.

37. Food properly labeled; original container
- comments: observed bulk containers of spices/flour with no labels. Instructed manager to label all container with common name and maintain.

38. Insects, rodents, & animals not present
- comments: observed one dead mouse in basement. Instructed manager to remove dead rodents, clean and sanitize affected areas.

49. Non-food/food contact surfaces clean
- comments: observed charred residue and food debris on exterior surfaces of cooking equipment. Instructed manager to clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed leak at bottom of 3 compartment sink. Instructed manager to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in toilet room. Instructed manager to provide and maintain

55. Physical facilities installed, maintained & clean
- comments: observed cooking equipment, supplies, boxes stored on floor in dine in area. Observed cans of paint and other supplies at bottom of stairs leading to back basement stairs. Instructed manager to remove unnecessary items, elevate items 6 inches off floor and properly store cooking equipment.

55. Physical facilities installed, maintained & clean
- comments: observed cracked floors and holes in floor behind 3 compartment sink and in dine in area along wall of hand sink. Instructed manager to seal holes to prevent entry of pests.

56. Adequate ventilation & lighting; designated areas used
- comments: observed ventilation hood with accumulated grease and no filter. Instructed manager to clean and replace filter.

57. All food employees have food handler training
- comments: observed several employees with no food handler certificates. Instructed manager to meet requirement and maintain.

59. Previous priority foundation violation corrected
- comments: previous priority foundation not corrected in specified time frame from inspection #2549482 dated 2/7/2022. Observed evidence of mice on premises. Observed over 30 mice droppings on floor along walls near oven in first prep area, 20 droppings on floor along bottom stairway leading to basement, over 100 droppings along back wall of unused serving line in dine in area, over 50 droppings at entrance to back staircase and over 100 droppings in basement on shelves and floor. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority violation. 7-42-090. Citation issued
