- comments: observed 15-20 rodent droppings in kitchen prep area along wall of 3 compartment sink and behind the refrigerator. Pest control company on site at time of inspection. Instructed manager to clean, sanitize and maintain. Priority foundation 7-38-020(a) citation issued

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed 7 out of 10 black city of chicago garbage cans overflowing with garbage and the lids are not secured. Instructed manager that all garbage can lids must be closed and secured to prevent rodent harborage. 7-38-020(b) citation issued

55. Physical facilities installed, maintained & clean
- comments: observed missing ceiling tiles in food receiving room/ breakfast eat-in area. Instructed manager to replace ceiling tiles and maintain.
