47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.

50. Hot & cold water available; adequate pressure
- comments: found improper hot water temperature at both staff washrooms at temp of 77. 3f and 78. 6f, hot water temperature at handwashing sink at both washrooms for residents in dining area at temp of 78. 7f, three-compartment sink in the kitchen hot water temperature 106. 5f to 108. 2f. Instructed manager proper hot water temperature at handwashing sinks must be at a minimum of 100. 0f or higher through a mixing valve or combination faucet. Hot water temperature at 3-compartment sink must be at least 110. 0f properly to wash, rinse and sanitize utensils and equipment. Priority violation:7-38-030(c). Citation issued.

51. Plumbing installed; proper backflow devices
- comments: must not use raw wood as a means of elevation for under two-door reach-in cooler. Must provide a non-absorbent material. Excess water inside the two-door reach-in cooler, repair cause.

54. Garbage & refuse properly disposed; facilities maintained
- comments: cement ground excess standing water and food liquid spillage under receptacles due to area is uneven and forming puddles of water. Flies around said water and receptacles. Repair cause

55. Physical facilities installed, maintained & clean
- comments: broken and missing baseboard wall tile in the kitchen by and behind ice machine and walk-in cooler. Replaced cement floor uneven with rough surface by the walk-in cooler and throughout the kitchen, surface must be smooth and easily cleanable. Missing wall tile inside women's staff washroom and nurse washroom(first floor). Broken cement and missing floor tile at mop sink first floor, at same area broken/holes on ceiling. Missing ceiling tiles in the kitchen above the prep table. Broken and heavily staine in dining area replace.

60. Previous core violation corrected
- comments: previous core violation not corrected from 10-8-2019, report #2315483 #55)6-501. 11 observing leaking faucet at 3 compartment sink on right-hand side. Instructed to repair and maintain. #38)6-202. 15 observed kitchen rear exit door and exit door not rodent-proof with gap about 1/4 inches. Instructed to seal gaps to prevent entry points for pests. Priority foundation violation:7-42090, no citation issued
