- comments: observed evidence of appx. 50 or more mice droppings on motor of unused cooler,lower cabinet at 1- comp sink,inside of fryer cabinet,on floor in corners of dry food storage shelves,on top of motor of unused dish machine,on floor behind crates of water and juice in storage area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/16/22. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of both lunch room doors and delivery door along right side. Must make doors tight fitting.

- comments: must store plastic wear with handles in upright position.
49. Non-food/food contact surfaces clean
- comments: must clean mold build up from walk in cooler shelving units and dust build up on fan guard covers in walk in cooler and wall fan in pizza prep area. Must clean grease and food debris build up from interior of fryer cabinets. Must defrost ice cream freezer.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a lid for garbage can in 2nd fl. Women's staff toilet room.
55. Physical facilities installed, maintained & clean
- comments: must repair or replace holes in walls outside of rear storage area.
