- comments: found 2-door prep cooler next to woks in cooks line area at improper temperature; 43. 3 f. Instructed to repair and discontinue use until the cooler maintains a proper cold holding temperature of 41f or below. Priority violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed over 10 live cockroaches in the kitchen, 10 in the lower level floor storage room, and approximately 10 in the basement storage areas. Observed over 25 live small flies in the upstairs bar, lower level mens washroom, and dry storage room. Management instructed to contact pest control for service and to clean/sanitize affected areas once pests are removed. Priority violation 7-38-020(a).

- comments: must remove all dead roaches and flies throughout the facility.

- comments: found multiple holes in the walls throughout the facility. Must repair/patch holes.
