Observed raw chicken at 50. 1 f, cut tomatoes at 50. 5 f, cooked potatoes at 46. 2 f, cooked beef at 98. 0 f, cooked noodles at 103. 4 f, tofu at 48. 3 f, and cooked chicken at 104. 3 f. Instructed to discard all said tcs foods and always maintain all cold holding tcs foods at 41f or below. Manager voluntarily discarded all said tcs foods. Priority violation 7-38-005. Citation issued.

- comments: observed found two 2-door prep coolers in the cooks line area at improper temperatures; 47. 5 f and 45. 0 f. Instructed to discard all tcs foods and do not use either cooler until they both maintain a proper cold holding temperature of 41f or below. Priority violation 7-38-005. Citation issued.

- comments: found no thermometer inside the 2-door prep cooler in the cooks line. Must provide.

- comments: observed over 20 live cockroaches in the kitchen, 50 in the 1st floor storage room, and approximately 50 in the basement storage areas. Also observed over 100 rat droppings in the 1st floor rear storage room and the basement storage areas. Management instructed to clean and remove all rodent droppings from the basement area and to remove all roaches and activity, clean/sanitize affected areas and contact pest control for service. Priority violation 7-38-020(a). Citation issued.

- comments: found heavy frost buildup inside 1-door upright freezer and 2 chest freezers in basement. Must defrost, clean, and maintain.

Must replace. Found a large hole in the wall in the first floor rear dry storage/garage room. Must repair hole.

- comments: floor in basement walk-in cooler is made up of unsealed concrete. Instructed management to seal floor so that it is non-absorbent, non-corrosive, smooth, and easily cleanable.
