- comments: observed approx:25 rodent(mice) droppings along the walls and corners and on top of boxes on storage racks also, in lower storage cabinets at front service line area. Instructed to detail clean and remove all rodent droppings and sanitize all affected areas; additional pest control service is now recommended. Priority foundation violation 7-38-020(a).

- comments: observed gap/opening at bottom right corner approx:1/4' mode of entry at rear kitchen exit door #9 leading to the trash dumpster. Must pest proof doors all across the bottoms to prevent pest entry and maintain.

- comments: observed stained cutting boards stored on kitchen storage rack. Instructed to resurface or replace the cutting boards and maintain.

- comments: observed water leak at drain pipe underneath the dish sprayer sink (located at rear storage room). Instructed manager to repair plumbing leak and maintain.

- comments: observed no covered waste receptacle provided in the kitchen employee washroom. Must provide and maintain.

- comments: instructed to replace water stained ceiling tiles in the serving line lunchroom area.
