5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on site to respond to vomit and diarrhea incidents that can potentially occur inside of the facility. Instructed to provide an adequate vomit and diarrhea clean up kit and written procedure on site at all times. Priority foundation. Citation issued 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12 observed the front food prep area hand washing sink not maintaining the required minimum hot water temperature, at the time of the inspection hot water only reach 72. 3f. Instructed to maintain 100f at all handwashing sinks at all times. Priority violation. Citation issued 7-38-030(c).

22. Proper cold holding temperatures
- comments: 3-501. 16 observed tcs foods at inadequate temperatures in 2 - 3 door prep coolers. Observed about 30 lbs of cooked chicken at 71f, about 3 lbs of raw beef at 49f, and about 1/2 lbs of raw eggs at 47f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at 41f or below at all times. Employee discarded said foods during the inspection. Priority violation. Citation issued 7-38-005.

28. Toxic substances properly identified, stored, & used
- comments: 7-201. 11 observed a can of toxic heavy duty grease cleaner being stored on the cutting board that is attached to the prep cooler in the food prep area. Instructed to never store toxic substances on food contact surfaces and always store toxic items in a separate area away from the food prep area. Employee removed the can during the inspection. Priority violation. Citation issued 7-38-005.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: 4-301. 11 observed two - 3 door prep coolers, with tcs foods inside, at inadequate cold holding temperatures; the first prep cooler is at 48f and the second one is at 54f. Said prep coolers tagged and held for inspection and may not be used. Instructed to maintain all refrigeration units at 41f or below at all times. Priority violation. Citation issued 7-38-005.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 30 rodent droppings scattered under the dry storage shelves located in the rear food prep area, about 10 rodent droppings located in the corner under the 3 compartment sink and about 30 rodent droppings also in the rear under steel shelving. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

40. Personal cleanliness
- comments: 2-402. 11 observed female food handlers not wearing hair restraints at the beginning of the inspection. Instructed to wear hair restraints at all times in the food prep area. Food handlers put on hair restraints during the inspection.

45. Single-use/single-service articles: properly stored & used
- comments: 4-502. 13 observed cardboard being reused in the prep coolers as a liner. Instructed to not reuse cardboard. All surfaces must be smooth and easily cleanable.

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed the interior of the fryers with accumulated grease dirt and food debris. Instructed to clean.

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 observed the front food prep area handwashing sink is slow draining. Instructed to repair so that said handwashing sink drains properly.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 observed floors and floor drains throughout with accumulated food grease and dirt debris. Instructed to clean and sanitize all.

58. Allergen training as required
- comments: 2-102. 13 observed no allergen training certificate available for inspector's review. Instructed to meet requirement for all city of chicago food service sanitation managers on site.
